BROCKVILLE – The regional health agency for Leeds-Grenville is surveying the population on elder care.
The South East Local Health Integration Network has launched an online survey to gauge feedback on its so-called Older Adult Strategy.
The two-page survey – geared to older adults, caregivers and agency workers – asks them about their input on the overall strategy and the priority of putting parts of the plan into action.
You can find the survey here.
The plan is split into five sections: focus on caregiver well-being, early identification of high-risk older adults, promoting and preserving wellness and functionality, enhancing care, and supporting those with dementia and mental health issues.
The survey results will be used to build an implementation plan, which will be presented to the LHIN board in June.
The South East LHIN covers an area from Brighton to Prescott and Cardinal and north to Perth, Smiths Falls and Bancroft.
